Strategic Planning & Program Evaluation
Program success is largely attributable to the early phases of planning, including the collection of data and information from a wide variety of sources. Program evaluation helps you understand whether your hard work is moving you in the right direction – are you doing what you intended to do, and is it producing the result you had wanted? You’ll need to think about your evaluation at the beginning of your program to ensure you’ll have the data you need at the end.

- Needs assessment helps you understand what is going on in the community, the assets that will be available to support your work and the barriers that will need to be overcome. A good needs assessment will help you determine areas to focus on, but will also give you information that will help you get started. We are happy to complete needs assessment on your behalf, but you might benefit most if we work alongside your staff under a coaching framework.
- Strategic action planning takes into account the needs assessment as well as organizational capacity, interest of stakeholders, funding availability, and practical considerations. Strategic planning processes can get messy when multiple organizations or a large leadership structure is involved in decision making, so it helps to have a well-facilitated process. We specialize in strategic planning for community groups, coalitions, and task forces.
- Program planning is the process of building objectives, budgets, timelines, steps, and partnerships around an identified goal. When planned well from the beginning, programs are more likely to run smoothly, on time, and on budget. We are available to work alongside your team to ensure you have a good understanding of every component of your program and are set up for success.
- Program evaluation can take many forms, depending on the type of program you have, your capacity and budget to complete an evaluation, and whether any data collection is already taking place that informs program outcomes. Through well-designed processes, training, and ongoing coaching, program staff can be empowered to complete many – or all – of the program evaluation tasks themselves. Organizations often need support for program evaluation when applying for large grants, as most grants have rigorous evaluation requirements. 2022 Rural Hospital Planning and Transition Grant Program (Minnesota)
The Minnesota Department of Health is seeking applications for its Rural Hospital Planning and Transition Grant Program.

The purpose of the Rural Hospital Planning and Transition Grant Program is to provide funds that assist small rural hospitals:
- develop strategic plans that preserve or enhance access to health services, or
- implement transition projects to modify the type and extent of services provided, based on an existing strategic plan.
Health Equity Priorities
In determining which hospitals will receive grants the following criteria shall be taken into account:
- improving community access to hospital or health services;
- changes in service populations;
- availability and upgrading of ambulatory and emergency services;
- the extent that the health needs of the community are not currently being met by other providers in the service area;
- the need to recruit and retain health professionals;
- the extent of community support;
- the integration of health care services and the coordination with the local community organizations, such as community development and public health agencies; and
- the financial condition of the hospital.
Funding Information
- Estimated Award Minimum: $25,000
- Estimated Award Maximum: $45,000
Project Dates Projects awarded funding will be approved for a period of one year (12 months). The anticipated start date of grant agreements for successful applicants is June 1st, 2022.
Eligible Projects
Grants may be used by hospitals and their communities to (1) develop strategic plans for preserving or enhancing access to health services. and (2) implementing transition projects to modify the type and extent of services provided, based on an existing plan. Grant funds may be used for COVID planning activities. At a minimum, a strategic plan must consist of:
- a needs assessment to determine what health services are needed and desired by the community. The assessment must include interviews with or surveys of area health professionals, local community leaders, and public hearings;
- an assessment of the feasibility of providing needed health services that identifies priorities and timeliness for potential changes; and
- an implementation plan.
Eligibility Criteria
Eligible applicants are small, general, acute care rural hospitals that are non-federal and not-for-profit, and that fulfill all of the following criteria:
- is either located in a rural area, as defined in the federal Medicare regulations, Code of Federal Regulations, title 42, section 405.1041, or located in a community with a population of less than 15,000, according to United States Census Bureau statistics, outside the seven-county metropolitan area;
- has 50 or fewer beds; and
- is not for profit.
