If you’re passionate and excited about your new business venture, you’ll be more likely to succeed. Success requires dedication and commitment, and that will be more likely if you’re doing something you enjoy. On days when commitment flags, think about why you started your business. That “why” will help you get up in the morning on tough days.

Find a problem you can solve. Successful businesses solve a problem that keeps their ideal clients awake at night. Hospitals help people heal when they’re ill. A health coach helps people achieve better health for more energy and vitality. A foot-care nurse helps patients perform tasks they aren’t able to do themselves. Staffing agencies help healthcare organizations solve staffing shortages. A good nurse consulting business saves people time and money or improves their health and well-being.

Identify your ideal client. When you identify whose problem you can solve, you can direct all your marketing efforts to them. Where do they work? What do they read? Where do they gather? For legal nurse consultants, for example, the ideal client is an attorney whose cases include medical issues. You can meet them in the courthouse, advertise to them in legal journals, and network with them at trial lawyer association meetings. Knowing your ideal client makes it easier to find them.

Ask your ideal clients to work with you. Early in my career, I asked my mentor how he gets all his clients. He said, “People want to help. All you have to do is ask!” Find those who need the services you offer and ask them to work with you and become your clients. Share the value and benefits of working with you so that they can choose to become a client. Another way to “ask” clients to work with you is through an online presence, which can be a website or a LinkedIn profile.

Offer packages. To move away from a dollars-per-hour model, offer packages for a flat fee. Packages can include products and services that will help people achieve the results they seek. For example, as a health coach, you can have 3-month or 6-month packages that include services such as an initial evaluation, one face-to-face meeting per month, and one phone consultation per week. This monetary commitment will help clients achieve better results than they would in a single session.

Don’t automatically choose certification. You have an RN license and the accompanying experience and skills, but do you also need a certificate to succeed in your new business? Certification costs time and money, so if your business requires only your nursing acumen, perhaps you can forgo certification, especially if your ideal client doesn’t find value in it. But if certification will help you acquire new skills specific to your business venture or is valued by your clients, look for a reputable accrediting body and budget your time and money wisely as you earn certification.

Begin with the end in mind. What’s your destination? What do you want your business to look like a year from now? How many clients do you want to serve? How much money do you want to make? How do you imagine spending your days? When you have your destination, you can build a road map to get there. If you want 10 clients, think about how many potential clients you need to connect with before 10 say yes. Having the end in mind is your barometer for how things are going. Don’t get discouraged if you’re not progressing as you anticipated. Try something new; many paths can get you to the same destination. To keep your planning focused, consider writing a short business plan that summarizes some of the points discussed here. (See Creating a map. )

Find a mentor. When you first started in nursing, you had a preceptor who showed you the ropes and taught you how to put your best foot forward. While the conversion to a direct pay and concierge medicine practice may be timely, a common question we receive at Concierge Medicine Today (CMT) is how to attain capital for starting a direct-pay or concierge medical practice. The answer lies in your planning and relationships with the appropriate types of people that can help you.

“In today’s economy, many have drained their savings accounts, and maxed out their personal credit lines,” say one concierge healthcare consultant. “Because of this they [doctors] need to reach out to others to get this money. The solution lies in coming up with a well thought out and professional, private-pay business plan for your practice that can be submitted to healthcare consultants, your attorney and a trusted accountant. Unfortunately, many people [doctors] have never learned how to write a proper business plan and immediately look to the Internet to search for a concierge medical practice business plan sample or template they can purchase. In my opinion, this is the wrong solution.”

TIP #1: Don’t Buy a Concierge Medicine Business Plan “Sample”

When purchasing a business plan sample from the Internet, people often force their concept into the boilerplate template rather than creating a plan that highlights it. A friend of CMT had recently made this type of purchase. He filled in the blanks and talked about it with us. Our first question was how he had determined that within his first five years he would have 15% growth annually. His unsure answer, “it was in the business plan sample.”

CMT has put together several resources and outlined below a few steps that should set you on a path to finding new patients for your new private-pay medical practice and hopefully to higher annual sales as well.

TIP #2: Interview Several Healthcare Consultants

There are certain areas that healthcare consultants and industry experts focus on to help physicians change their business model from an insurance and managed care practice to a direct-pay, cash-only or retainer-based business model. For example, when assessing your practice for potential economic viability and success in the marketplace, your current patient-base makeup, local demographics, patient surveys and your bedside manner all factor into the review process. . Most available ‘canned’ business plan templates don’t cover these types of intangibles critical to your to success.

Recently, CMT attended a seminar in Florida that was designed to help physicians learn more about direct-pay medical business models. When the speakers explained to the attendees that this type of practice business model is highly relational, one physician raised his hand unapologetically and said, ”so you’re telling me I have to be nice to my patients.” Shockingly, there was nothing funny about it.

TIP #3: Set Realistic Expectations for Your Time

When you sit down to start writing your plan, you must remember that this is a business model based on service and fulfillment. If you are clear with your patients about what it is that you are going to deliver to them in the form of medical services, it will be much easier to introduce the program concept to your patients at the right time, with the right message.

A long-time reader of CMT based in Atlanta, GA (ranked number 5 as the most popular places for concierge medicine practices in the U.S.) told us that the reason she was leaving her concierge physician was because for nine months she was seen by her doctor only one time. Otherwise, every other visit she was treated by the nurse practitioner.

Those that use the excuse that they cannot write a business plan are the same people who have never thought out all of the aspects of starting a direct-pay, concierge or retainer-based practice. In other cases they may have thought out the service or business aspects, but have not taken the time to identify and understand information voids that exist in their planning process. Questions your private-pay business plan should answer:

  • What local alliances or relationships can you leverage with other businesses to help yours?
  • In terms of fees, what can the local market/demographic support?
  • How will the skills of your staff, their knowledge of the industry and track record of implementation support this shift?
  • What existing problem or problems exist that your practice is trying to solve simultaneous to the shift in practice and business model?
  • What solutions does your practice have in place to resolve the problem(s)?
  • How much will it cost to solve these problems now?
  • How will solving these problem(s) make your practice financials look in one, three and five years, when considered in the context of your shift toward a private-pay practice model?
  • How much cash do you need to find a path to profitability?

TIP #4: Choose Your Model. Then Define it and Make it Unique to You, Your Area and Your Patients.

This becomes another stumbling block doctors will run into. Doctors call CMT monthly and want to learn more about the various nuances of this industry. They quickly learn that there are several attractive business models that could be implemented and be suitable to a specific demographic and geography. But you must write a business plan with a practice model in mind that is suitable for your local area and demographic makeup.

The business plan you write for your future practice must make sense to those whom you will serve. Unfortunately, many consultants prefer one medical practice model over another, so it’s important to choose a consultant to with experience across various business models and knowledge of proven models that have been successful in similar locales. Be careful, there are a lot of companies out there that know nothing about this industry.

TIP #5: Cash, Insurance, Medicare. What plans, if any, Will You Participate In?

Several years ago, CMT wrote a story that’s been published many times over and has become one of the most copied and popular reads online about the difference between direct primary care and concierge medicine. Direct primary care (DPC or sometimes called direct care) is a term often linked to its companion in health care, ‘concierge medicine.’ Although the two terms are similar and belong to the same family, concierge medicine is a term that fully embraces or ‘includes’ many different health care delivery models, direct primary care being one of them.

Similarities Direct care practices, similar in philosophy to their concierge medicine lineage, bypass insurance and go for a more ‘direct’ financial relationship with patients and also provide comprehensive care and preventive services for an affordable fee. However, direct care is only one branch in the family tree of concierge medicine.

Direct care, like concierge health care practices, removes many of the financial barriers to ‘accessing’ care whenever care is needed. There are no insurance co-pays, deductibles or co-insurance fees. Direct care practices also do not typically accept insurance payments, thus avoiding the overhead and complexity of maintaining relationships with insurers, which can consume as much as 40 cents of each medical dollar spent, according to CMT analysts.

Differences Direct care is a ‘mass-market variant of concierge medicine, distinguished by its low prices.’ Simply stated, the biggest difference between ‘direct primary care’ and retainer based practices is that direct care takes a low, flat rate fee whereas concierge medicine models (although plans may vary by practice) typically charge an annual retainer fee and promise more ‘access’ to the doctor.

Both health care delivery business models are providing affordable, cost-effective health care to thousands of patients across the U.S.

“This primary care business model [direct primary care] gives these type of providers the time to deliver more personalized care to their patients and pursue a comprehensive medical home approach,” said Norm Wu, CEO of Qliance Medical Management based in Seattle, Washington. “One in which the provider’s incentives are fully aligned with the patient’s incentives.”

One additional word of advice: once you have written your private-pay medical practice or concierge medicine business plan, do as many smart physicians have done, have it reviewed and read by a trusted friend in business or a relative. After they have read it, have them give you a verbal explanation as to how they think your new practice model will work, based on your plan. If they do not understand the plan or cannot explain the practice model concept from what you have provided, there is a very good chance that a patient in the coming months ahead will not understand the practice concept(s) either.

Your competitive analysis should identify the indirect and direct competitors your business faces and then focus on the latter.

Direct competitors are other concierge services businesses.

Indirect competitors are other options that customers have to purchase from that aren’t direct competitors. This includes individuals who prefer to complete tasks on their own, or from dedicated personal assistants.

With regards to direct competition, you want to describe the other concierge services businesses with which you compete. Most likely, your direct competitors will be concierge services businesses located very close to your location.

With regards to the last two questions, think about your answers from the customers’ perspective. And don’t be afraid to ask your competitors’ customers what they like most and least about them.

The final part of your competitive analysis section is to document your areas of competitive advantage. For example:

  • Will you provide a wider range of services?
  • Will you offer greater convenience, such as online requests?
  • Will you provide personalized customer service?
  • Will you offer better pricing?

Think about ways you will outperform your competition and document them in this section of your plan.  

Marketing Plan

Traditionally, a marketing plan includes the four P’s: Product, Price, Place, and Promotion. For a concierge services business plan, your marketing plan should include the following:

Product : In the product section, you should reiterate the type of concierge services company that you documented in your Company Analysis. Then, detail the specific products you will be offering. For example, in addition to arranging dinner reservations, will your concierge services business provide dining recommendations and reminders of upcoming anniversaries or other important dates that typically include dining out?

Price : Document the prices you will offer and how they compare to your competitors. Essentially in the product and price sub-sections of your marketing plan, you are presenting the services you offer and their prices.

Place : Place refers to the location of your concierge services company. Document your location and mention how the location will impact your success. For example, will you operate from a physical office, or will you interact with clients online and/or at their home or place of business? In this section, discuss how your location will affect demand for your services.

Promotions : The final part of your concierge services marketing plan is the promotions section. Here you will document how you will drive customers to your location(s). The following are some promotional methods you might consider:

  • Advertising in local papers and magazines
  • Reaching out to local websites
  • Signs and billboards
  • Social media marketing
  • Local radio advertising

Operations Plan

While the earlier sections of your business plan explained your goals, your operations plan describes how you will meet them. Your operations plan should have two distinct sections as follows.

Everyday short-term processes include all of the tasks involved in running your concierge services business, including marketing, providing consultations, completing tasks, maintaining company vehicles, keeping up to date on restaurants and events, etc.

Long-term goals are the milestones you hope to achieve. These could include the dates when you expect to assist your 100th person, or when you hope to reach $X in revenue. It could also be when you expect to open a concierge services business in a new location.  

Management Team

To demonstrate your concierge services business’ ability to succeed, a strong management team is essential. Highlight your key players’ backgrounds, emphasizing those skills and experiences that prove their ability to grow a company.

Ideally you and/or your team members have direct experience in managing concierge services businesses. If so, highlight this experience and expertise. But also highlight any experience that you think will help your business succeed.

If your team is lacking, consider assembling an advisory board. An advisory board would include 2 to 8 individuals who would act like mentors to your business. They would help answer questions and provide strategic guidance. If needed, look for advisory board members with experience in customer service, or successfully running small businesses.  

Financial Plan

Your financial plan should include your 5-year financial statement broken out both monthly or quarterly for the first year and then annually. Your financial statements include your income statement, balance sheet and cash flow statements.

Income Statement : an income statement is more commonly called a Profit and Loss statement or P&L. It shows your revenues and then subtracts your costs to show whether you turned a profit or not.

In developing your income statement, you need to devise assumptions. For example, will you serve clients on a contract basis, or on an as-needed basis? And will sales grow by 2% or 10% per year? As you can imagine, your choice of assumptions will greatly impact the financial forecasts for your business. As much as possible, conduct research to try to root your assumptions in reality.

Balance Sheets : Balance sheets show your assets and liabilities. While balance sheets can include much information, try to simplify them to the key items you need to know about. For instance, if you spend $50,000 on building out your concierge services business, this will not give you immediate profits. Rather it is an asset that will hopefully help you generate profits for years to come. Likewise, if a bank writes you a check for $50,000, you don’t need to pay it back immediately. Rather, that is a liability you will pay back over time.

Cash Flow Statement : Your cash flow statement will help determine how much money you need to start or grow your business, and make sure you never run out of money. What most entrepreneurs and business owners don’t realize is that you can turn a profit but run out of money and go bankrupt.

In developing your Income Statement and Balance Sheets be sure to include several of the key costs needed in starting or growing a concierge services business:

  • Office build-out including fixtures, construction, etc.
  • Cost of buying or leasing a company vehicle
  • Cost of office supplies such as software
  • Payroll or salaries paid to staff
  • Business insurance
  • Taxes and permits
  • Legal expenses

Attach your full financial projections in the appendix of your plan along with any supporting documents that make your plan more compelling. For example, you might include your office lease, or an overview of all the services you provide.

While doctors have traditionally been the ones to offer concierge care, today nurses are jumping on the boutique bandwagon as well. Target Market Segment Strategy

GCS intends to target people who need service during the day but do not have the time to perform necessary tasks. We are focusing on corporations/companies because they have a large number of employees who would find a concierge service attractive as a part of their compensation package, and the employer would prefer to pay our service to perform the employee’s task, allowing the employee to stay at work and remain productive. Even though we will offer the companies a discount over our standard hourly rate, our revenue will continue to increase because we will see an increase in usage without the normal costs of attracting the individual customers. We would bill the corporation monthly, decreasing the labor costs for processing the invoices.

We will also be targeting individual clients. This targeted group will be a much smaller percentage of our business but will be charged a higher hourly rate. This group will be targeted to provide us with work when we have vacancies in our schedule from the corporations.

Service Business Analysis

The concierge industry in Eugene is relatively small. It is currently serviced by two different companies that cater primarily to individual clients. Currently, there are two competitors in the concierge service space. These services are listed under “errands & miscellaneous service” headings of the yellow pages and they target individuals as opposed to corporations.

Competitive Edge

Our competitive advantage will be based on two factors: our concentration on corporate/company clients, and our investment in human capital.  Since we are concentrating on the corporate market, we will be able to best meet the needs of this market. We will become aware of what these special needs are and offer them. These special needs are flexibility, monthly billing, and the ability to handle a decent number of transactions.

Our other competitive advantage is based on our investment in human capital. GCS will be developing a fairly extensive training program to provide employees with the tools necessary to perform at a level that the founder alone would otherwise be offering. It can be difficult to maintain a high level of customer service if you are relying on employees. This problem can be overcome and turned into a competitive advantage by providing employees with the tools to offer the same high level of service that GCS provides. While this costs money, and takes time up front, it will pay off in the long run. This is based on our mission statement that it is our goal to attract and maintain customers. It is difficult to expect a mission statement which demands high levels of service to become organizational behavior unless you provide the organization with the tools to achieve this high standard.

Sales Strategy

Our sales strategy will be targeted at obtaining the corporate/company clients. It is our belief that the individual customers will be obtained through word of mouth. It is likely that they will have spoken to a previous client about GCS and the referral of our services will speak for itself.

Our sales strategy will be to use a cost benefit analysis when trying to close the sale of a prospect. We will use quantitative information to illustrate the value the company will receive by offering a concierge service to employees. The value is recognized in different forms:

  • The attraction and retention of employees who are offered this service.
  • The value in keeping the employee productive at work instead of taking care of personal maintenance outside of work during normal business hours.

Additionally, we will provide the company with several different options in which they could offer this service to employees ranging as a no-cost-to-the-employee perk to a partially-subsidized service. The different options will be attractive to the company based on a number of different variables, from the cost they are willing to invest into the program to the amount they are willing to spend on the different employees within the company.

Our sales strategy for individuals will be based on our Web presence. Through our marketing efforts we will be driving people to our website. Once on our website, people will see the wide range of services that we offer and can then contact us for a service request. The website will be especially useful for out-of-town clients who are in need of a Eugene-based concierge service.

Please note that GCS will be accepting credit from the corporations. This is based on the assumption that the corporations would find this option a major benefit so that they only had to reconcile their account once a month, as opposed to every time a transaction occurs.

Sales Forecast

The first month will be spent setting up the business. It is unlikely that there will be much sales activity. In addition to dealing with legal and accounting issues for the business, equipment will need to be purchased and set up. Once the physical office is set up, we will then develop a manual for future employees. The manual will cover topics such as company policies and procedures, the training program (which covers ways to handle clients, tips on efficiencies, principles of empowerment, etc.), and other necessary kernels of information. This will take time so it is important to take care of this initially before the real business gets underway, otherwise the document would be likely produced under time constraints affecting the quality of the document.

The second month will begin to see some activity. The clients will be served by the founder, as there will not yet be a need to bring on new employees. During these few months, at least half of the administrative details (scheduling, answering phones, etc.) will be done from remote locations while serving clients. The remainder of the administrative details will be completed at the office. After month five, GCS will bring on another employee to serve the clients, and the founder will then be able to perform the bulk of the administrative details from the office. Month seven will see the addition of a second employee, and by month 12 we will plateau at three employees, allowing Taylor to remain in the office 95% of the time (of course there will be exigent circumstances requiring Taylor’s services in the field). Having the ability to operate the office remotely (with a pager and cellular phone) will provide the necessary flexibility to accommodate a wide range of needs.

The first employee will be needed by the fifth month, as GCS will be receiving at least six hours of jobs a day. This, coupled with required administrative time, will necessitate an employee. By month seven we will see an increase in our corporate program and will utilize a second employee. By month 12 we will likely be at a level that will plateau for the foreseeable future. Years two and three will be similar to the activity in month 12.

GCS will have several milestones early on:

  • Business plan completion. This will be done as a road map for the organization. While we do not need a business plan to raise capital, it will be an indispensable tool for the ongoing performance and improvement of the company.
  • Office set up.  This will done within the first month.
  • Training program.  This will be the development of a training program for the employees. The employees will receive initial sessions of training all at once, and then training will become an ongoing process. Continual learning is key.
  • Quarterly training.  This is basically a check up to make sure that the employees are still learning skills and developing.

An extensive insurance policy should cover any liability GCS is exposed to.

Taylor Gogetter, founder and owner, has a degree in philosophy from Washington and Jefferson College. Taylor spent four years after college managing a retail bicycle shop. As manager, one of Taylor’s principle responsibilities was attending to customer needs. While 40% of his time was devoted to the administrative details to running the company, 60% of the time was dealing with customers, assessing their needs and delivering a service that exceeded their expectations.

Taylor also spent two years guiding hiking trips of 12-18 people through the Alps on two-week trips. In this experience, Taylor spent 30% of his time accomplishing administrative tasks with the remainder 70% spent addressing the needs of the customers.

Prior to these experiences, Taylor opened and operated his own Sealcoating business. Taylor was able to develop close relationships with customers and was awarded jobs based on his superior services.

These experiences, taken as a whole, provide Taylor with a wealth of practical knowledge to develop a high quality service organization. Taylor’s recognition that it requires time and money to properly train employees is essential to the prosperity of GCS, as the employee will be the interface that the client deals with.

Personnel Plan

The staff will consist of Taylor working full-time for GCS. We will hire one full-time employee at $9 per hour in month five, a second full-time employee in month seven, and a third in month 12. GCS has decided to compensate its employees at $9, which is a fair amount above minimum wage. This wage rate was chosen to attract a higher-quality employee than what minimum wage typically attracts, it will also increase employee morale, and decrease turnover. This is important as, after month five, it will be the employees who are representing GCS and there is a need to exceed the clients expectations every time. GCS will provide an extensive employee training program on an ongoing basis. GCS is cognizant that it costs far more to train a new employee relative to maintain a current employee.
